If we determine that you are right for this program, you can download the intake form, fill it out and send it back to me. Afterwards,we can schedule an appointment for a 1 hour Health Evaluation to dig deeper.

I want to dive deep in to your story and spend some time with you to go into greater detail about your health goals and health history so that we can determine which labs are necessary to run for your individual needs.  I will also talk a little more about the program, the support system you need to have in place prior to starting and throughout the duration of the program, and what you can expect during our time together.

Please make sure to fill out and submit the intake form prior to scheduling Health Evaluation. The Health Evaluation fee is non-refundable. If you sign up for the Essentials Program, the $147 fee will be applied to the cost of the program.
